Please download and complete the form using the link below, and return to us, together with details of the the proposed data collection tool, to uk.notch@nhs.net.

We encourage submissions from multiple/collaborative authors. It is helpful to have the results of a pilot study available with your proposal submission, but is not mandatory.
We find that simple yet well considered projects are often the most successful; please note this when completing your proposal form.
